In-Store Purchases:

1. Date of the purchase (& the approximate time if you know!)
2. Exact Amount of the purchase ($)
3. Location of the Five Below store you visited (https://locations.fivebelow.com/)
4. Last 4 Digits of the Credit Card (if items were purchased with cash, please write CASH)
5. Some of the items purchased (this will help us to locate the right receipt)
